Use of this referral form is the first step in the process for Second Chance Dayton Group Home to consider your client for our Residential Therapeutic and Behavioral Health Program/Treatment services. Submission of this form will be followed by a phone call.

Our admissions team is skilled in helping families determine what level of care is appropriate for our clients. If you’d like an in-person meeting to determine what level of care is needed; we offer face-to-face assessments at our private clinical office or we will come to you. - Do you suspect you or the person you’re referring is using drugs?
- Do you think you or your person you’re referring is experiencing mental health issues?
- Do you or person you’re referring need more support with therapy/outpatient treatment?
- Have you recently discovered drug paraphernalia in you or person you’re referring’s possession?
- Have you or person you’re referring stopped engaging in activities they used to love?
- Have you or person you’re referring started to withdraw from the family?
- Have you or person you’re referring in process of transitioning from an outpatient, intensive outpatient, or residential treatment program?
- Have you or person you’re referring been unsuccessful in outpatient or intensive outpatient programs?
- Are you or person you’re referring showing signs/ and or feeling stress, depression, anxiety, anger, emotional disturbance, grief, of any sort?
